Excellent opportunities exists for motivated and experienced Nurses to join our growing team.

The Western Health at Home teams deliver care in the community to patients in their own homes and residential aged care facilities.

The aim of the service is to:

  • provide an alternative to hospital presentation and admissions
  • effectively coordinate the care of people with Chronic and complex conditions
  • promote and support safe and early discharge from inpatient settings
  • provide support for patients to remain at home following a change in health or functional ability and
  • provide an urgent response service to clients who become unwell in their home
Primarily, the position is a responsible for delivering high quality nursing care within the at home Nursing service. You will have recent, relevant and exemplary nursing skills, demonstrate a high degree of flexibility and be able to work in a busy environment, enjoy working independently and be able to prioritise.

To be successful in the role you will have:

Essential
  • Registration as a Registered Nurse in Australia
  • Demonstrated effective clinical skills appropriate to the Community role
  • Demonstrated ability to work with a diverse and complex professional workforce.
  • Demonstrated ability to consult and collaborate with others and work as an effective member of a team to deliver organisational outcomes
  • A demonstrated commitment to innovative flexible models of care and service delivery
Desirable
  • Experience in working within the community nursing setting with recent acute care experience.
  • Knowledge and understanding of clinical and patient data information systems
