Multiple opportunities exists for experienced and enthusiastic Clinicians and Nurses to join the Western Health at Home teams within the Chronic and Complex Care Division.

The Western Health at Home teams deliver care in the community to patients in their own homes and residential aged care facilities.

The aim of the service is to:
provide an alternative to hospital presentation and admissions

effectively coordinate the care of people with Chronic and complex conditions

promote and support safe and early discharge from inpatient settings

provide support for patients to remain at home following a change in health or functional ability and

provide an urgent response service to clients who become unwell in their home

The role of the Care Coordinator is essential to the Western Health at home team in providing clients and carers’ with coordination and management of their chronic illness and navigation of the Health and Primary care system. The Care Coordinator is responsible for comprehensive patient assessments, providing self-management support and coordinating patient healthcare needs across the health care continuum, through a variety of mediums including, face to face consultation, phone, telehealth and the use of client management systems.

The Care Coordinator works within a multidisciplinary team model of care and contributes to team based care plan development in collaboration with the client. They will also play a vital role in ensuring that patients are actively engaged with their GP in establishing Advanced Care plans that are available for utilisation in the acute care sector.

Skills & Experience or Essential criteria

Appropriate tertiary qualification as a Registered Nurse or Allied Health Professional and eligibility for registration with relevant body

Minimum 5 years clinical experience in relevant acute/community setting

Previous Care coordination experience

Strong leadership skills, including proven interpersonal and communication skills

Demonstrated ability to work in multidisciplinary teams
