    Right now, we're looking for an Client Services Coordinator to join our Home Support Services team.

    About The Role

    Reporting and being supported by the Group Manager, Home Care Services, you’ll be primarily responsible for identifying, on-boarding and coordinating client CHSP, Private and Home Care Package services.

    The Client Services Coordinator will work amongst a multi-disciplinary team to provide integrated care that promotes healthy ageing and wellbeing, social engagement, prevents frailty and avoidable decline and achieves reablement for clients to optimise their ability to live better in their own home.

    About You

    You will be suited to this role if you have a positive mindset and can prioritise well. You’ll have the ability to demonstrate integrity, calmness and personal resilience. Passionate about person-centred care, enabling clients to maintain maximum independence, you will be able to seek creative solutions in your approaches to overcome any barriers.

    All Southern Cross Care employees must have a current National Police Certificate for the Purpose of Aged Care.
