JOB DESCRIPTION

An opportunity exists for an experienced and enthusiastic Grade 2 Social Worker to join the growing Western Health at Home teams within the Chronic and Complex Care Division.


The Western Health at hometeams deliver care in the community to patients in their own homes and residential aged care facilities.


The aim of the service is to:



  • provide an alternative to hospital presentation and admissions

  • effectively coordinate the care of people with Chronic and complex conditions

  • promote and support safe and early discharge from inpatient settings

  • provide support for patients to remain at home following a change in health or functional ability and

  • provide an urgent response service to clients who become unwell in their home


The Grade 2 Social Worker will provide high quality care to clients through the use of proficient clinical skills, effective communication, inclusive practices and participation in the organisation and development of staff, (including teaching and supervision), within the Chronic and Complex Care division.


Duties



  • Deliver effective clinical assessment and intervention for clients referred to Western Health at home programs

  • Supervise students as part of the University teaching programs

  • Be proactive about identifying and addressing issues that interfere with patients receiving Best Care

  • Ensure safe and high quality patient care is delivered in line with Western Health values and policies


Skills & Experience



  • Recognised degree or equivalent in chosen discipline and current AHPRA registration or equivalent

  • Relevant post graduate qualification desirable

  • Extensive experience in assessment of people with complex needs

  • Highly developed interpersonal, written and electronic communication skills

  • Commitment to continuous quality improvement, clinical research and professional development of self and others

  • Strong understanding of community services

  • Demonstrated ability to work in multidisciplinary teams

  • Current Victorian driver’s licence

  • Current working with children’s check
