Assessment And Review Officer | Aged care Services | Community Services | 3 months contract | Geelong, VIC

Your new company
Hays in Geelong are currently partnered with a large State Government organisation located in Geelong, Victoria.
We are seeking to appoint an Assessment And Review Officer to join organisation’s Aged Care Regional Assessment team for an initial 3 months with a view of further extension. This role is required to work in Service Coordination and Regional Assessment Services to do an assessment of older persons in the community through the My Aged care portal. Assess for clients’ needs and referrals to appropriate services.

Your new role
In this role, you will responsible for ensuring that the assessed needs and delivery of services to eligible frail aged people, people with dementia, people with disabilities and their carers within our community, are met in a timely manner in accordance with Council’s policies, DHHS Service agreements and Assessment and Service Delivery Conflict of Interest Business Unit Procedure. You will be providing and updating service data via the My Aged Care online provider portal and Carelink+. Assess the workplace safety for Community Care Workers who will be undertaking the care plan in the clients’ home and report any potential risks.

What you'll need to succeed

To be considered for this role, you will have:
A minimum of Diploma in Aged Care, Community Services or other tertiary qualification in Human Service field is required.
Work experience and knowledge of issues relating to frail aged people, people with dementia, people with disabilities and their carers.
Awareness and support of the philosophies of healthy ageing, promotion of independence, non-intrusive care and general disabilities.
Extensive knowledge of governmental and community resources available for older people, people with dementia, people with disabilities and their carers.
Comprehensive knowledge of holistic assessment and care management practices.
Program planning and development skills.
Knowledge of funding, HACC guidelines and administration MAC systems, and Council policies and procedures is highly desirable.
Please note: Due to the nature of our client, for this role you MUST be an Australian Citizen OR a Permanent Resident living in Australia
