Position Summary

We're seeking a Care Coordinator at our Labrador facility to support exceptional care delivery for our residents!

Working with your Residential Service Manager you’ll utilise your skills and experience to drive outstanding care for our residents. With brand new leadership team and service delivery structure, there's never been a better time to join Blue Care.

You’ll have the opportunity to lead and inspire your team providing supervision, guidance and mentorship, through your effective decision making and clinical experience. You’ll provide exceptional care whilst responding to clinical nursing issues and concerns. With their wellbeing always in mind, you will review resident care and care plans promoting independence & quality of life, drive ACFI funding and make a real difference to the quality of care we deliver. This includes:

Development, implementation, monitoring and review of care plans
Comprehensive and accurate assessment of residents
Assisting with the orientation of new staff and ongoing professional development
Support in ensuring the service meets all accreditation standards

What you’ll need:
You’ll be an experienced nursing clinician, with previous experience as a Clinical Lead or Clinical Nurse within a health, community or aged care environment. You’ll be an advocate for person-centred care, leading by example through exceptional communication and customer service. You’ll present with:

Minimum of 3 years residential aged care experience as an RN
Current AHPRA registration
Ability to commit to senior clinical on-call requirements (generally 4 week off / 1 week).
A good understanding of ACFI
Experience coordinating care provision within a large team
Confidence to manage conflict, pressures and competing priorities
Ability to provide leadership, support and mentorship to other team members
