Permanent, part-time, day work, working 6.4 hours per fortnight commencing ASAP.

Duties

As a member of the Midlands Multi-Purpose Health Centre (MMPHC), the Leisure and Lifestyle Coordinator is responsible for:

Coordinating Leisure and Lifestyle programs for the permanent Residential Aged Care residents of the MMPHC.

Planning, implementing, and evaluating quality individual and group resident programs at the MMPHC.

In the role of Leisure and Lifestyle Coordinator you will -

Consult with residents, carers, health care providers and community members in planning, developing, implementing, monitoring, and evaluating quality individual and group programs.

Work with residents, their families and nursing staff to identify and plan individual leisure and lifestyle programs.

As part of a multidisciplinary care team, proactively liaise with the Clinical staff and MMPHC Multi-Disciplinary Team and report on activities undertaken including changes in the health status of the residents.

Essential Requirements

The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. * The following checks are to be conducted:

1. Conviction checks in the following areas:
a) Crimes of Violence
b) Sex Related Offences
c) Serious Drug Offences
d) Crimes involving dishonesty

2. Identification check

3. Disciplinary action in previous employment check

  • as required by the Aged Care Act 1997, occupants of this role are required to undertake a conviction check assessment every three years.
Desirable Requirements
Completion of a recognised course in Leisure and Lifestyle and/or equivalent qualification/s.

Current Drivers Licence.

Current First Aid Certificate.
