The town of Cloncurry is regarded as the "Gateway to the Gulf", the birthplace of the Royal Flying Doctor Service and QANTAS; and is located approximately 120 kms east of Mount Isa.

About the role

Cloncurry is a Multipurpose Health Service (MPHS) that has 15 acute care beds, 10 Aged Care beds, Community Health Centre, X-Ray department and Pharmacy. Visiting services include Child Health, and Immunisations, working closely with Central West Queensland Medicare Local, Centacare and Blue Care. A range of visiting specialists include Paediatrician, Obstetrician and Gynaecologist, General Surgeon, Woman's Health Nurse, Renal Nurse Practitioner.

The Discharge Planner will:
  • Provide clinical and professional practice in relation to patient flow and discharge facilitation for the Cloncurry MPHS.
  • Effectively coordinate and manage all aspects of the discharge planning process for patients, families and staff within the Cloncurry MPHS.
  • Apply advanced level decision making and professional and clinical expertise in collaboration within a multidisciplinary environment.
Are you the right person for the job?

A Bachelor of Nursing, or equivalent, qualification, registration with the Nursing and Midwifery Board of Australia (AHPRA) and possession of a current annual practising certificate are mandatory requirements for this role.
  • Ability to lead service planning and delivery across the care continuum and achieve against unit performance and relevant standards.
  • Sound knowledge of contemporary nursing practice and theory and the ability to apply relevant legislation, guidelines and standards.
  • Proven ability to develop and maintain effective communication that works collaboratively with colleagues, patients and their carers.
  • Ability to integrate the strategic direction and priorities for quality improvement into the clinical service, establishing a quality framework.
  • Well-developed interpersonal, written and oral communication skills including the ability to effectively use problem solving, change management and conflict resolution frameworks.
  • Proven ability to use information and reporting systems appropriate to the position.
  • Ability to demonstrate ongoing professional development of self and others.

  • This is a Vaccine Preventable Diseases (VPD) risk role.
  • Pre-employment screening, including an Aged Care criminal history and discipline history checks, may be undertaken on persons recommended for employment.
  • Employees who are permanently appointed to Queensland Health may be required to undertake a period of probation appropriate to the appointment.
