About the role

In collaboration with the multidisciplinary team (internal and external) the Clinical Care Coordinator (CCC) provides assistance to patients and their families in coordinating planned discharge destinations and supports to ensure timely, safe and appropriate services are arranged and provided in the community on discharge in accordance with the Calvary Mission and Values.

The CCC will provide direct patient contact throughout the patients’ episode of care. The CCC will also support unwarranted care variations and reliability in care by:-

- Early assessment and intervention as per the endorsed criteria

- Clear communication with patients, carers, relatives, clinical team(s) and all key stakeholder

- Wise stewardship aligned with expected accountability of a Level 2 Registered Nurse

- Clinical support to Clinical Managers, Team Leaders and the wider clinical teamThe

- The Cardiac CCC will also encompass the role of diabetes educator across Calvary Adelaide Hospital and achieve compliance with the specialised clinical care standards.

What you bring
