Juniper Community provides community based care programs that assist our aged clients to continue to live independently in their own homes. The Senior Client Care Advisor is responsible for the management and delivery of CHSP, HACC and HCP within a Consumer Directed Care framework. This role exists to help ensure these services are person centered, responsive and support clients to live as well and independently as is possible in their own home and community. To help achieve this purpose, the Senior Client Care Advisor will provide effective support and leadership to the team of 11 Client Care Advisors (CCA’s).

On offer for the successful candidate is a full time position working 75 hours per fortnight.

Key Responsibilities:

  • Support the team of CCA’s to manage CHSP, HACC and HCP programs to ensure appropriate services are delivered that meet client needs.
  • Develop a solution-focussed and continuous improvement culture and support CCA’s in the resolution of client care issues and problems.
  • Staff leadership and development including training, performance monitoring and conducting performance appraisals.
  • Build strong relationships with referral sources and the wider multi-disciplinary team to facilitate the ongoing growth of Juniper and delivery of quality services to clients.
  • Identify and resolve operational issues as they arise.
  • Ensure compliance with Occupational, Safety and Health requirements including appropriate Injury Risk Management.

Demonstrated Skills, Knowledge and Experience required:

  • Tertiary qualifications in human services or equivalent experience in Community aged care (essential).
  • Cert IV qualifications in Frontline Management, Community Services or Aged Care (essential).
  • Coordination / case management of a community based care program preferably aged care.
  • Staff leadership experience including team coordination, training and performance management.
  • Experience in managing budgets and interpreting financial information.
  • High-level of computer literacy and knowledge of Procura is highly desirable.
  • Warm and engaging demeanour with excellent written and verbal communication skills.
  • A customer focussed attitude and the ability to build trusting and respectful relationships.
  • A current and valid WA Driver's Licence.
