About the role

The Client Care Manager is responsible for the service development and ongoing case management of Calvary Community Care (CCC) disability clients, including on-boarding new clients, the co-design of care plans, and the evaluation and continuous improvement of service delivery outcomes.

The Client Care Manager is the designated point of contact for all clients, their families and/or representatives by providing a responsive service through the pre-engagement and ongoing service delivery process to ensure efficient and effective service delivery, and the monitoring of planned services which match the CCC Service Excellence framework, and other legislative frameworks and standards.

What you bring

  • Relevant tertiary qualification in Community/Disability/Psychology/Counselling/Mental health
  • Experience in Aged/Disability/Community sector.
  • Flexibility in the spread of hours required from time to time in order to meet business requirements.
  • Strong background in lean principles, quality, safety and risk.
  • Ability to assess and develop strategies to support people living with disability in accommodation and community settings.
  • Experience working in a customer care environment, working with clients and families to provide best practice and flexible, outcome focussed service delivery.
  • Experience in identifying new sources of business with the ability to convert these to clients in consultation with relevant stakeholders.
  • Knowledge and understanding of reporting requirements, identifying risks and monitoring and managing client funding, annual budgets and growth targets.
  • Proficiency in the use of software packages including databases, MS Office and CRMs with the ability to learn and adapt to new systems.
  • Commitment to Calvary’s philosophy of providing responsive, flexible and customer focused services.
  • Team player with the ability to develop and maintain effective high level working relationships across a diverse range of business, local/state/national government, employees and client.
  • An insured car and license essential
  • A valid/current DCSI check in disability services employment or willingness to get one at your own expense.
