At Mercy Community, we support a broad range of people that interface with multiple agencies including significant work in the mental health, corrections and child safety space.

The NDIS offers funding to people with a broad range of physical, intellectual and psychosocial impairments. Some of these people have complex challenges that reduce their capacity to utilize their NDIS funding in the best way possible. This is where you come in!

You will be responsible for working directly with NDIS Participants and other networks to connect with and maintain a variety of relationships which will enable them to implement their NDIS Plan and achieve their goals and aspirations.

We currently have a Specialist Support Coordinator role available based in Cairns, if you’re looking for full-time or part-time work on a permanent or fixed term basis, we’d love to hear from you.

A typical day in the life of a Specialist Support Coordinator can include:
  • Support participants, their families and their formal/informal support network to understand their NDIS plan;
  • Complete and engage in regular communication/reports with the participants, their families and NDIA;
  • Identifying the most appropriate services within the community to suit participants needs;
  • Support participants to communicate their goals and aspirations to their stakeholder team and the NDIA;
  • Attend home and community visits with participants;
  • General case management of participants; and
  • Manage and monitor participants NDIS Plan budget.
About You…
  • You will be working towards or have completed a tertiary degree in Social Work, Psychology, Human Services, or a related discipline;
  • Demonstrate high level of case management, care coordination and/or complex case coordination;
  • Highly developed adaptable and person-centered communication skills;
  • Strong connections with disability, mainstream and community networks within the identified region;
  • Positive and friendly personality, who is open, flexible and willing to be part of an innovative and supportive team who work together;
  • Have an open Queensland Driver’s License; and
  • Yellow Card Exemption/NDIS Workers Screening Clearance and Blue Card or ability to obtain prior to commencement.
