As a Registered Nurse with Juniper Community, you will be responsible for assessing clients' needs in conjunction with other healthcare professionals, and for the planning, implementation and evaluation of their care plans. Reporting to the Clinical Services Manager, this position provides professional nursing assessment, practice and evaluation in a clinical setting. A willingness to work within a person-centred, enabling environment, that respects the rights and needs of each individual is essential for success in our organisation.

This position will be responsible for visiting clients in their own homes across the Perth Metropolitan region. Our Community team is located within the Juniper Rowethorpe site in Bentley. On offer is a permanent position working a minimum of 15 hours per week. Days and times will vary so we are seeking someone who is fairly flexible with the ability to also be on call when required.

Key Responsibilities of this role:

  • Assess client needs in participation with other professionals
  • Plan, develop and implement client care plans and provide clinical care and advice to clients
  • Respond to the requests and concerns of families and Community Support Workers
  • Actively involves clients and significant others in the planning of care and related goals
  • Liaise with appropriate health care professionals to deliver quality care to clients
  • Develop and maintain the effective performance of other care and service staff
  • Ensure full compliance with and maintenance of all company and aged care standards

The Skills, Knowledge and Experience required:

  • Registration with the Nursing and Midwifery board of Australia
  • Qualifications and/or experience in a clinical specialty - e.g.: wound care, palliative care etc. (desirable)
  • Demonstrated experience working in a community care environment will be highly regarded
  • Knowledge and experience in the provision of clinical care
  • Planning and co-ordinating skills in matters related to delivery of care
  • Excellent written and verbal communication skills along with the ability to adapt and solve problems
  • WA Driver's Licence and flexibility to travel across Perth metro areas
  • Sound knowledge of medication systems
