About the role


The Social Worker is responsible for the provision of Social Work services within the GEM@Home program. The Grade 2 social worker is expected to provide high quality social work intervention to clients and families through advanced clinical skills, knowledge and effective communication. The clinician will also demonstrate autonomy of practise and professional leadership through proactive participation within the multi and/or interdisciplinary team.


The Grade 2 Social Worker is expected to supervise and mentor students and junior staff members as requested. The Grade 2 Social Worker is expected to initiate and participate in teaching, training, research and quality activities. They may also represent their discipline or team at various internal/external forums.



This position may be required to work across all Eastern Health sites and will include weekend work.



About you



To be successful in this role you will:



  • Demonstrate advanced skills in comprehensive psycho-social assessments and effective interventions.

  • Demonstrate capacity to function independently and articulate interventions and plans to a multi-disciplinary team.

  • Demonstrate understanding and experience of social work supervision

  • Demonstrate understanding of health care systems and aged care.

  • Have well developed communication and interpersonal skills to promote teamwork, and a proven ability to relate effectively to people at all levels of the organisation.

  • Demonstrate participation in professional development relevant to the position.

  • Have Established skills in organisation, planning and priority setting.

  • Have well-developed understanding of professional codes of conduct and ethical practice.

  • An AASW accredited Bachelor Degree or entry level Masters Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers.
