SYDNEY CARE PARTNERS is passionate about providing home and community care service to people requiring aged care service, disability service, mental health and carer support services. We are a progressive and friendly young team that provides a range of flexible services to support people to live independently in their own homes and the community.

The Role

We are seeking a caring and hard-working individual to work with our clients. The role of a Support Worker is to provide high quality support service to ensure our clients are receiving the best of care, happy with our workers and wanting more services from SYDNEY CARE PARTNERS.

Key Responsibilities

To provide high-quality care to our clients based on their individual needs. This includes assisting them with:

  • Social support and interaction - to support client increase confidence and access in the community
  • Daily living skills - support clients with activities that support them to function independently and have some fun
  • personal care assistance - to support client maintaining personal hygiene and feel good and be proud of themselves
  • domestic assistance - to ensure clients have a nice and clean space to live in and be proud of their own space
  • mobility and transport clients - support clients with getting to services that helps them maintain wellness and well-being e.g. medical appointments or social outings such as shopping, dancing, attend art classes, meet and interact with other like-minded clients to achieve common goals

To support clients to participate in age appropriate activities to achieve their every-day life goals and maximizing quality of life for each client every day.

To provide excellent customer service experience every day.

To develop and maintain a respectful and professional relationships and networks with clients, families, caregivers and key stakeholders

To Be Successful

Positive attitude, hard-working, caring nature and friendly at all times.

Demonstrated ability to work collaboratively with individual clients and their families, adopting a person-centred approach

Ability to work proactively towards targets and goals set by Rostering Officer

Relevant experience and willingness to work with vulnerable people

Police Record Check, Working with Children Check

Driver’s licence, comprehensive insurance along with a registered, roadworthy vehicle.

Qualification in Disability, Aged Care or equivalent (or be working towards obtaining one)

Ability to work various sessions (day, afternoon, night, sleepover and active nights) throughout the week and weekends. Preferably be available over a 24/7 for on-call
