JOB DESCRIPTION

ABOUT THE ROLE:

Complete comprehensive assessments of older people in hospital and community settings in order to facilitate access to appropriate government approved care services based on clients’ needs

  • Fixed Term part-time contract 32 hours per fortnight until June 2022
  • Comprehensively assess the care needs of frail older people, and facilitate access to care services by determining eligibility for a range of Australian Government care options.
  • Interest in learning the ACAT assessment process and learning reablement principles.
  • Ability to work independently and efficiently, and utilise online reporting programs to meet assessment time frames and targets.
  • Teamwork, communication and advocacy skills are required to liaise with multidisciplinary team, care providers, community services and families.

ABOUT YOU:

  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as applicable
  • Minimum 1 years post graduate experience as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist
  • Previous experience working with geriatric population
  • Excellent assessment and literacy skills and confident adapting to computer programs
  • Current unrestricted Driver’s License and ability to drive distances for work purposes
  • Strong time management and prioritisation skills required
