Community Allied Health within Central Coast Local Health District is seeking a suitably qualified Level 1/2 Social Worker for a community based position as part of a multidisciplinary allied health team. The position provides intervention in the home and clinic based settings and is involved in providing education as part of the chronic disease group programs. This position is a permanent position and an eligibility list will be created for further temporary positions that may arise. 

What you'll be doing The position of Social Worker is part of a community based multidisciplinary allied health team. Community Allied Health provides a community service approach seeing clients and their families across the Central Coast. This position will work within a dedicated interdisciplinary team to deliver high quality care in a dynamic team environment- providing social work intervention to support clients of Community Allied Health, as well as many specialist teams across CCLHD (such as Specialist Palliative Care Service, chronic disease programs & Integrated Care Co-ordination team).

Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.
This position is required to work collaboratively with other social workers within the team to ensure appropriate and timely service provision across all geographical sites and services. The clinical duties are performed in a range of settings, including home, residential aged care facilities(when indicated), clinics, groups and residential aged care facilities (when appropriate), and the role can be based in any of the community health locations across multiple sites within CCLHD.
