Casey Manor has 45 aged care residential beds and is located in Narre Warren South, Victoria. We pride ourselves on delivering the best care for our residents and firmly believe that happy staff equates to happy residents.

This is a fabulous opportunity to work in our residential home as the second in charge of the home to the manager with an excellent team and high staff morale.

About the Role ...

The Clinical Care Coordinator is the senior clinician in charge of the home and leads and supervises the team of professional staff including registered nurses, enrolled nurses and personal care workers. The Coordinator provides empathy and support to residents and families, best practice holistic care, clinical expertise and care planning and maintains Accreditation Standards for the home.

The Clinical Care Coordinator will have demonstrated knowledge of ACFI, a high standard of interpersonal communication skills, including experience with admission assessments, liaising with GP's, family members, and also overseeing the management of administration of medications, accurate documentation to ensure a high standard of care.

This is a Monday to Friday position full time.

About you ...

You are a highly organised and self-motivated individual with excellent clinical knowledge of aged care - someone who can manage their time and be flexible with their workloads and you lead by example. You'll have a high level of skills in mentoring your team and a focus on internal leadership and empathy and compassion too.

To be successful in this role you would need to possess the following: 

Essential skills and competencies:

  • Bachelor of Nursing Degree
  • Current registration AHPRA Registered Nurse
  • Work experience in a leadership Registered Nurse position of at least 5 years in an aged care setting
  • Teamwork with colleagues
  • Friendly and positive personality
  • Exceptional Clinical care knowledge and skills 
  • Experience liaising with visiting GP's, allied health providers and conducting resident & family meetings or supporting meetings with family members
  • Leadership and mentoring of team members
  • Excellent interpersonal communication skills and customer service
  • Ability to build a strong positive working relationship with your manager
  • A high level of competency in computer and clinical documentation skills
  • Ability to smoothly run a shift and the home and manage a variety of issues
  • Demonstrated practice of Hygiene and Safety practices in aged care
  • A good understanding of the Aged Care Standards, documentation requirements, accreditation and ACFI requirements
  • An understanding of continuous improvement and OH&S
  • Current police check
