The Company

Our client is a market leading provider of aged care services across Australia. Based in Western Sydney, they are currently looking for an experienced Case Manager to join the team on a temporary basis.

The Role

Based in the Parramatta office you will be responsible for coordinating services for clients who are eligible for home care packages. This includes client assessment, coordinating delivery of services and updating the client and other providers.

This role is available for an immediate start and may extend. The office is easy to access with free on street parking available.

Who You Are

You will be an experienced Case Manager and demonstrate assessment skills and the ability to problem solve who has worked in a similar capacity in an Aged Care setting. Strong customer service skills and organisational skills are essential, as is prior experience using Procura.

A tertiary qualification or certified training relevant to Aged care, Community Services, Social Science, Nursing or Allied health if is also essential.
