What we are looking for….

A Support Coordinator (Identified) based in Cairns to work with NDIS participants and contribute to making their lives better.

You are part of the Aboriginal and Torres Strait Islander community.

You will be passionate and interesting.

You will provide and facilitate pathways for NDIS participants to meet their NDIS goals.

You will coordinate services from community organisations to help NDIS participant’s physical, intellectual and social needs.

A typical day in the life of a Support Coordinator can include:

  • Support participants and their families to understand their NDIS plan
  • Regular communication/reports with the participants, their families and National Disability Insurance Agency (NDIA)
  • Engage with community organisations to see if their services are a good fit for the participants
  • Prepare NDIS plan reviews to communicate participants goals and aspiration
  • Home and community visits
  • Managing cases – we will show you
  • Refer to the NDIS price guideline to check service agreements – we will coach you
  • Culture is part of every day.
About You…
  • Experience in community services and engagement and ability to network with community organizations.
  • Knowledge or ability to acquire knowledge of managing cases, care coordination and/or complex case coordination.
  • Adaptable and person-centered communication skills.
  • A positive and friendly manner, flexible and willing to be part of an innovative and supportive team
  • Completion of NDIS Worker Orientation Module
  • Open Queensland Driver’s License
  • Ability to obtain a Yellow Card Exemption/NDIS Workers Screening Clearance and Blue Card prior to commencement
Please note: to perform this role it is essential that the person be an Aboriginal person or Torres Strait Islander person, this is a genuine occupational requirement under section 25, 104 and 105 of the Anti-Discrimination Act 1991 (Qld).
