The Federation of Polish Organisations in SA Incorporated runs HCP, CHSP and other aged care programs for our seniors to assist client "Wellness and Reablement", delivering services ranging from home care through to meals, transport assistance and social support to give dignity, compassion and care enabling seniors to live independently in their homes and engage with their community.

We’re looking for qualified caring disability services officers who are passionate about caring for older people and helping them to live independently – in their own homes. Our clients are in a wider Adelaide area, therefore an appropriate, reliable vehicle is necessary.

Brief description of the role:

Reporting to the HCP Manager your primary role is to support and enable older people and people with disabilities to maintain and optimise their independence where it is their choice to live at home and receive “Home Care Services” as well as those registered with the short term and emergency program “In Home Respite

Main tasks and responsibilities:

  • Provide personalised support in a sensitive and confidential manner, to individuals and families according to their personalised support plan
  • Understand and follow support plans, seeking clarification from the Manager if necessary
  • Monitoring and reporting on the progress of clients
  • Promote self-care and self-management, to achieve a maximum level of independence
  • Medication administration
  • Work in partnership with members of the multidisciplinary team and report any changes in a Client's mental or physical health, or social conditions, to the relevant professional
  • Providing community inclusion and networking opportunities for customer and supporting customer’s leisure activities
  • Communicate with and acknowledge the Circle of Support as part of the Client's Support team, and ensure that any changes in its' members' health and wellbeing is reported to the multidisciplinary team
  • Assist in the implementation of strategies and policies to meet the rights and responsibilities of participants 
  • Assisting customers in all aspects of personal care 
  • Contribute to learning and development, including by demonstrating an on-going commitment to learning and by sharing.
