We have an exciting opportunity for a suitably qualified and experienced Home Care Packages Case Manager to join our amazing Community Health team.

About the Role

  • Part time - 16 hours per week
  • Ongoing

As a Home Care Packages Case Manager you will work collaboratively with clients, their families/carers and care staff to ensure we are providing a high level of care and services to those who need a range of assistance in their homes.

About You

To be successful in this role you will have:

  • A health related degree, diploma or other community services qualification relevant to the role
  • Understanding of issues relating to the frail and/or elderly, people with disabilities, their carers and families.
  • Excellent written and verbal communication skills
  • Ability to build rapport with clients, carers, families and colleagues
  • A genuine desire to deliver best care, every person, every time
