Care Designer (Clinical Care Manager)

Nurse Next Door is on the lookout for an experienced Registered Nurse to join us in the role of Care Designer (you may know it as a Case Manager or Clinical Care Manager).

Operating in a homecare environment, you’ll be responsible for ensuring that our clients are safe and happy in their own homes. You’ll liaise with potential clients, setup individualised care plans to meet their unique needs and build professional relationships in the healthcare industry.

Your Next Adventure

Reporting to our General Manager, your role will be to help manage a growing team of caregivers and nurses, and will include but will not be limited to the following:

  • Design tailored and meaningful client care plans that foster positive relationships between our clients and the Care Team.
  • Meet with potential clients to consult on their options for premium homecare services.
  • Oversee client clinical care and maintain meticulous care documentation records.
  • Be accountable for maintaining the high clinical standards set by Nurse Next Door.
  • Assist in the management and development of a growing team of caregivers and nurses.
  • Believe in Making Lives Better® for our clients every day, through thoughtful care and support.

Mandatory Requirements

  • AHPRA Registration.
  • Registered Nurse.
  • CPR & First Aid Certification.
  • Car & Current Victorian Drivers License.
  • National Police Check (Aged Care/ Vulnerable Persons Sector).
  • Working With Children Check.
  • Disability Workers Scheme Exclusion Approval.
  • This permanent full-time position requires some flexibility and availability to work occasional evenings, weekends and on-call as required.

Part-time hours: 38 per week

Job Types: Full-time, Part-time, Permanent

Salary: $80,000.00 – $110,000.00 per year
