About the role

As a Support Coordinator you will provide coordination and transition support for individuals living with a disability. You will empower participants to make independent choices and take control over their own care and assist them to access a range of community supports, identify opportunities for innovative service delivery, break down barriers and resolve any concerns relating to service delivery.

  • Meet with NDIS Participants in relation to their NDIS plan
  • Discuss individual goals and make timely referrals in accordance with those goals and that of their funded plan
  • Report to NDIS and ensure the services are supporting client effectively
  • Strengthen participants understanding of NDIS Framework and empower autonomy for them to coordinate and direct support within their plans

Skills and experience

  • Tertiary qualification in Social Science, Social Work, Welfare or similar
  • Background in Community Services and understanding of the role of a Support Coordinator
  • Strong knowledge and understanding of NDIS principles, the Disability Act and other relevant legislation
  • Highly developed written and verbal communication skills
  • Ability to work collaboratively across the team and maintain professional relationships with stakeholders
  • Current Victorian Drivers Licence
