About The Role

The Case Manager is responsible for delivering case management that is person centred and enablement focused, in accordance with Calvary Community Care policies and procedures, program guidelines, the National Standards of Practice for Case Management and the National Code of Ethics and CCC's Case Management Model. The role will also include aspects of Care Coordination, including administrative tasks and rostering of clients services.

The Case Manager will develop positive working relationships within the local service centre and more broadly across the organisation to facilitate improved outcomes for our clients.

We are currently seeking an experienced full time Case Manager to join our Calvary Community Care team in our Sydney (Ryde).

What You Bring
  • Experience in delivering quality, client focused case management.
  • Understanding of the various funding sources available in the aged care sector.
  • Tertiary qualifications in Community Services / Case Management or related disciplines.
  • High level of computer systems proficiency, including data entry.
  • Strong prioritisation and time management skills.
  • Experience developing care plans for complex clients.
  • A current and valid driver's licence.
  • A desire to join a team committed to the care of others in a mission based organization
