We are a designated state-wide provider for services including trauma, and we lead centres of excellence for tertiary services in several key specialties including neurosciences, nephrology, oncology, cardiology and virtual health.

Your new team

RMH@Home Subacute is an innovative inpatient bed substitution program that is modelled on a rehabilitation and Geriatric Evaluation and Management (GEM) patients. RMH@Home Subacute provides a seven day specialist medical, nursing and allied health service to assist patients to rehabilitate within the comfort of their own home. We are a team working together to deliver excellence in patient care, research, clinical supervision and student teaching.

We enjoy a collaborative working environment with all members of the multi-disciplinary team to maximise care for out subacute RMH patients in their homes. The aim of this service is to provide delivery of patient centred safe and effective care As a Clinical Care Co-ordinator part of you role may be assisting staff on the road with questions or queries regarding patient care and /or assessing patients suitability for the service and managing an appropriate waitlist. You will be required to liaise with the medical and allied health team and foster a cohesive team environment.

Your success in this role, will be in your ability to

Ensure excellence in clinical care provision

Develop priorities for innovation and improved service delivery

Develop and maximise use of clinical governance and infrastructure

Enhance clinical partnerships and collaboration

Lead Quality Improvement planning and implement initiatives in line with organisational requirements within the RMH@Home service

Create and maintain an appropriate waitlist

Liaise with Nursing/Medical and Allied Health developing strong partnerships with the acute sector

Assist with assessing and accepting patients appropriate for the service

Manage referrals to external providers

Your Contribution

Extensive experience in discharge planning, team leadership and care-coordination

You are an influential leader with a track record of driving change within a complex healthcare environment and fostering the growth and development of the wider team.
