Working in a fast-paced environment, the successful applicant will form an integral part of NovaCare’s Intake Team. This role is responsible for the coordination of the consumer intake process in a manner that recognises the importance of assisting consumers live their way in their own homes.

About the Role…

We are looking for someone who is empathetic, has a strong ability to build rapport and proactively resolve queries whilst providing a high level of service to each consumer. You will be required to visit new clients in their homes to establish what services they require, set care plan goals, inform on budgetary requirements and ensure all necessary paperwork is filled and signed correctly to get services started.

Although this is not a nursing position, this position may suit someone with experience in residential/community aged care or someone from an allied health background.

What you will be doing -

  • Managing the initial support requests and proactively communicate and respond to consumers so that they feel well supported to access all available NovaCare resources
  • Coordinating the intake of clients including creating contracts and following through to execution of the contracts
  • Promoting the internal customer pathway between funding types ensures NovaCare’s ongoing sustainability.
  • Balancing conflicting priorities in order to manage workflow, ensure the completion of essential projects, and meet critical deadlines.

What you need to succeed -

  • You will pride yourself on your relationship building and ability to engender confidence in the capability and competence of our NovaCare team.
  • You will have exceptional customer service skills and be consumer focused.
  • You will be driven to assist clients by being responsive and following up on their requests
  • You can work autonomously and independently but also as part of team.
  • You have strong administrative and computer skills.
  • You will have an understanding of Home and Community Care Packages.
  • You will possess a knowledge of continuous improvement, accreditation requirements and documentation.
  • Current NSW Driver licence
  • Access to a reliable car with comprehensive car insurance
